How to Get an International Certificate of Vaccination for Polio

Do you have a scheduled trip to Indonesia? Brunei? Or Morocco?
You may as well first secure an international certificate of vaccination (ICV) from the Department of Health’s (DOH) Bureau of Quarantine (BOQ).
According to BOQ’s website, administration of polio vaccine and issuance of the ICV has a fee of Php300.
When you go there, you need to present valid identification cards (IDs) that may include any government-issued IDs or passport.
Then you need to fill-up the Polio Vaccination Form.
Booster shots for Oral Polio Vaccine (OPV) or Inactivated Polio Vaccine (IPV) are given to adults as required by the World Health Organization (WHO).
According to WHO’s website, the additional dose of OPV or IPV must be administered within four weeks to 12 months of travel.
Should a traveller wish to get an IPV from a private doctor, he or she may do so provided a signed medical certificate is secured that includes the name and lot number of the vaccine administered to the traveller, according to a report by Rappler.
The same fee of Php300 will be paid for the issuance of an ICV.
Apart from Indonesia, Brunei, and Morocco, other countries that require travellers an ICV are Georgia, Oman, Qatar, Lebanon, Maldives, Pakistan, and Seychelles.
